Bug: /memory should list grouped in-use rules and skills

Description

The /memory view should show the instruction files OpenCode is actively using and group them by source so it's easier to understand where context is coming from. This includes in-use AGENTS.md, Claude-compatible rule files, and loaded SKILL.md files, with separate groups for global, root, and nested rule files.

Steps to reproduce

1. Open the TUI in a repo with root and nested AGENTS.md files plus skill directories.
2. Run /memory.
3. Review whether all active memory files are listed and grouped clearly.
